Definition: An individual treatment plan is a written document identifying the specific problems that bring a teen into residential treatment and defining measurable goals for change. It identifies the problems to be addressed, how they will be addressed and spells out expected changes in terms of behavior.
The treatment plan is developed using information provided by the parents, the teen and assessments completed by all the professional staff providing services. It is closely monitored and adjusted as needed by staff working with your teen.
Parents are encouraged to review their teens' individual treatment plan in the first weeks of treatment and to expect regular updates.
